Data Protection legislation
Data Protection legislation is in force from 25th May 2018 and I want to reassure you about how I will handle any data you intend to share (or have shared) with me.
The General Data Protection Regulation (GDPR) is EU wide legislation which is currently enacted into UK law (being known as the Data Protection Act 2018.)
The data collected on you
A Consultation Treatment Form is used to record your personal details and medical history which includes your name, address, date of birth, and other generic contact details.
This personal data is not shared with anyone for any purpose whatsoever without your prior consent and is subject to strict confidentiality. It is held very securely within my clinic and is not entered into any database, online service, or platform but is simply retained in relation to my accessing your specific treatments records or to potentially contact you regarding appointments.
You have Individual Rights under the Data Protection Act 2018
- To be informed about the personal data I hold on you.
- To access your personal data.
- To object to the processing of your personal data.
- To restrict the processing of your personal data.
- To rectify your personal data.
You can exercise your Individual Rights at any time without charge. However, if your request is considered repetitive, unfounded, or excessive a reasonable administration fee can and will be charged.
I will take all appropriate technical and organisational steps to protect the confidentiality, integrity, availability and authenticity of your data.
Record Keeping and the GDPR
Personal data should not be kept for longer than is necessary. In the case of a client's treatment notes, records will be kept for 7 years after the last treatment. In the case of minor's records these will be kept for 7 years after they reach the age of 18.
